Parents/Guardians are asked to complete the following forms during the first 2 weeks of the school year. 

1. Reunification Form (Will be distributed to students - digital copy below) - Required
2. Personal Information Verification Form (Will be distributed to students - individualized) - Required
3. Medical Information Sheet (Print the file and bring to office if your child has medical condition) - Required
4. Windermere Field Studies Neighbourhood Consent (Will be distributed to students - digital copy below) - Recommended
5. CASL VSB Consent (https://myforms.vsb.bc.ca/) - Recommended
6. VSB Media Consent Form (https://myforms.vsb.bc.ca/) - Recommended
7. External Media Consent Form (https://myforms.vsb.bc.ca/)
8. Notice of Later Return Form (file below) - If not returning to school until after September 7th. (Must be completed by September 7th)
