Program Objectives

The Sunny Hill School Program provides educational programming for school-aged children and youth from across British Columbia and the Yukon Territory. Students participating in the Sunny Hill School Program are receiving services from one of two patient care programs at Sunny Hill: The Acute Rehabilitation program, which provides comprehensive treatment, assessment, and rehabilitation from an interdisciplinary team, and the Outpatient Neuromotor Program, which provides consultative services to children and youth with neurodevelopmental disabilities and neurological impairments/conditions. Consultations with the Outpatient Neuromotor Program are given shape through a guiding consultation question, which also informs the focus for individualized educational programming provided by the School Program.

Referral Process

Student referrals are received from the Acute Rehabilitation Inpatient Program or the Outpatient Neuromotor Program. Following the referral, a signed school consent to release information form is obtained from the parents/guardians of the student.

Discharge and Transition Planning

Upon discharge from the Acute Rehabilitation program, a Transition Report is made available to district and school personnel, with the signed consent of the parents/guardians, to ensure a seamless transition back to the school setting. Individuals are discharged from the Outpatient Neuromotor Program after assessments have determined that the consultation question has been satisfactorily addressed. Again, a report, termed an Interdisciplinary Consultation Report, is made available to district and school personnel, with the signed consent of the parents/guardians to ensure a seamless transition back to the school setting. The Sunny Hill Program teacher liaises with the student’s School or District Case Manager for the purpose of collaboration and to share educational recommendations.
