Post-Secondary Transition - Community Transition Program
Program Description
This is a pilot program designed to support student’s transition to adult life. The program is a full time community based program with an emphasis on employment, community inclusion, and life skills that are meaningful and related to the individual needs of each student. Person-centred planning is used in planning goals and objectives to suit the unique needs of each student. All students participate in on-going work/volunteer experiences and activities that foster community inclusion and participation.
Students must:
- be eligible for CLBC services
- be able participate in community based program-this is not a centred based program
- be in their last year of high school (turning 19 between July 1-June 30th)
The goal of this program is for an increased level of independence and participation in community based settings. Students who complete this program, continue with CLBC with a wide range of support options.
Referral Process
Currently, this is a pilot project and referrals must be submitted to the District Resource Teacher - Secondary Transition. There are parent/student information meetings in the late Fall of each year and application packages are given out at the meeting. Students are accepted based on meeting criteria and on a first come first served basis.
